Use the form below to request repair services for all Jetboots Systems and Jetboots related equipment. Please include as much information as possible to identify the equipment and the issues you are experiencing. Including the original purchase order number and serial numbers help us expedite your request and determine warranty eligibility. 


DO NOT include your Jetboots Battery when returning equipment for repair.

If your Jetboots Battery becomes damaged and needs repair, please include that information in the request form and Patriot3 will provide instructions. If Patriot3 approves the return of your battery, the shipment must be prepared by a Certified Dangerous Goods Specialist and conform to all IATA shipping regulations. Any unauthorized battery shipments will be declined and returned at the owners expense.

  1. Fill out the form and include as much detail as possible.

  2. Patriot3 will contact you within 24 hours with shipping instructions and a document containing a RMA number.

  3. Include a copy of the completed RMA document with the shipment.

  4. Write the RMA number clearly on the outside of the carton.
